Digital GLP-1 questionnaire

New and existing patients can use the digital GLP-1 questionnaire to send contact, medical-history, treatment and progress information to Praxis Jona for personal physician review. The data processed includes name, date of birth, email address, telephone number, weight and treatment goals, health and medication information, previous treatment progress, consent and signature.

Processing for medical review and treatment preparation is based on your explicit consent under Art. 6(1)(a) and Art. 9(2)(a) GDPR and, where treatment is initiated or provided, under Art. 9(2)(h) GDPR in conjunction with Section 22(1)(1)(b) BDSG. Your information is transmitted to Praxis Jona through a protected server-side workflow. Incomplete questionnaires are not stored permanently in the browser.

All information is personally assessed by a physician. No automated treatment decision, prescription or rejection takes place. Internal technical review flags serve only to support the physician's review. Data is deleted when it is no longer needed for medical review or treatment unless statutory documentation or retention duties require continued storage. Consent can be withdrawn at any time with future effect through the contact details stated above.

Meta Conversions API for job advertisements

When you arrive at a job posting on our website through a Facebook or Instagram advertisement and actively select “Apply by email,” we send a server-side conversion event to Meta Platforms Ireland Limited, Merrion Road, Dublin 4, D04 X2K5, Ireland. The event contains the time of the click, the job posting viewed, the IP address, browser identifier, and the click ID Meta appended to the advertisement link. We do not send application documents, names, email addresses, or health data.

We use only the Meta Conversions API. We do not load the Meta Pixel or set Meta cookies on our website. Organic job-posting visits without a Meta click ID are not sent to Meta. Processing is used to measure and optimize our job advertisements and is based on our legitimate interest in effective recruitment under Art. 6(1)(f) GDPR. Facebook, Google+, YouTube, etc.

Our website is also active within social networks and platforms. When you access the respective social media services, the terms and conditions and data processing policies of their operators apply. Unless otherwise stated in this privacy notice, we use and process data within social networks.

Data collected from patients, contractual partners, or interested parties is processed under Art. 6(1)(f) GDPR so that we can provide our contractual or pre-contractual services. The type, scope, and purpose of the processed data are determined by the contractual relationship. In principle, this includes:

  • name
  • address
  • email
  • phone number
  • contract data, such as costs, products, contact persons, and services
  • financial data, including bank details

Data covered by Art. 9(1) GDPR, such as health information, requires the patient's explicit consent under Art. 6(1)(a), Art. 7, and Art. 9(2)(a) GDPR.

Data collected for preventive healthcare is processed under Art. 9(2)(h) GDPR and Section 22(1)(1)(b) BDSG. Explicit consent is also required here. Where necessary, collected data may be disclosed or transmitted to third parties within the limits of statutory provisions and for the purpose of fulfilling the contract, for example to further medical treatment providers, health insurers, laboratories, or similar recipients under Art. 6(1)(b) GDPR and Art. 6(1)(c) GDPR. This may happen where legally required, where necessary to protect another natural person, or on the basis of consent under Art. 6(1)(a) and Art. 7 GDPR.

Data is deleted after fulfillment of our contract and statutory duties of care. Warranty obligations and comparable obligations remain unaffected. We review data and deadlines every three years. Statutory retention periods otherwise apply.
